Overview for ºÚÁÏÍø³Ô¹Ï±¬ÁÏHealth Professional Colleges

In addition to the university's vaccination requirements, listed on Immunization Compliance Overview page, ºÚÁÏÍø³Ô¹Ï±¬ÁÏHealth Professional Graduate student may have additional requirements. Per ºÚÁÏÍø³Ô¹Ï±¬ÁÏPolicy 33-002, it is mandatory for ºÚÁÏÍø³Ô¹Ï±¬ÁÏstudents to submit all required immunization documentation and/or the completed ºÚÁÏÍø³Ô¹Ï±¬ÁÏMedical History Form prior to course registration. Course registration will be restricted until you have fulfilled this requirement.

Tip: Your previous institution may provide you with copies of your vaccination documents or can document your vaccinations on our ºÚÁÏÍø³Ô¹Ï±¬ÁÏMedical History Form.
