How to Submit a Degree Verification Form Request

If you graduated from the 黑料网吃瓜爆料College of Education and are requesting a Degree, Program, or Institutional Verification Form for an employer in another state, please note the instructions below:

Please complete any personal information requested on the form (such as name, address, birthdate) then email it as a PDF to the applicable contact listed below. If the information below is not required on the form, please add it in the text of the email when sending the PDF:

  • Your name as it was when you graduated
  • Your 黑料网吃瓜爆料ID (U Number) and/or date of birth
  • Where the form should be sent upon completion
